"Microsoft Forms" User Guide

Logging in and Creating a Form

1. Go to Microsoft Forms

Visit http://forms.office.com.

2. Sign in with Your KU Account

Use your KU email (name.surname@ku.lt) to log in (see Fig. 1).

3. Create a New Form

Click the "New Form" button to start creating a form (see Fig. 2).

4. Enter the Form Title

Type the desired form title in the input field (see Fig. 3).

Creating Form Questions

1. Add Questions

Click "Add new" (or "+") to add questions (see Fig. 4).

2. Choose a Question Type

Available question types (see Fig. 5):

  • Choice (multiple-choice selection)
  • Text (short or long text answer)
  • Rating (star or number rating)
  • Date (date selection)
  • Likert (scale response)
  • File Upload (allows file submissions)

3. Adjust Question Settings

For each question, you can (see Fig. 6):

  • Mark the question as required.
  • Add additional information (subtitle).
  • Set answer restrictions.

Sharing the Form

1. Collect Responses

Click "Collect responses" in the top menu (see Fig. 7).

2. Choose a Sharing Method

Available sharing options (see Fig. 8):

  • Link
  • QR Code
  • Embed (for websites)

3. Configure Security Settings

Available options (see Fig. 9):

  • "Anyone can respond" (open access).
  • "Only people in my organization can respond" (KU members only):
    • Record responder's name ("Record name").
    • Limit to one response per person ("One response per person").
  • "Specific people in my organization can respond" (restricted access).

Viewing Responses

1. Directly in Microsoft Forms

  1. Open the form.
  2. Navigate to the "Responses" section (see Fig. 10).
  3. View:
    • Summary statistics
    • Individual responses
    • Graphs

2. Viewing Responses in Excel

  1. Choose one of the options (see Fig. 11):
    • "Open results in Excel" (view in browser).
    • "Download a copy" (download responses).

Ensuring Security

  • Use organization login.
  • Restrict access to KU members only.
  • Avoid public links for sensitive data.
  • Regularly check sharing settings.
